Overview:

As a Home Care Nurse, you will be responsible for providing comprehensive in-home medical care to patients within their residences. This role involves assessing patients' health needs, developing care plans, and administering treatment in coordination with the healthcare team and the patient's family.

Responsibilities:

• Perform initial patient assessments, including physical examinations and health histories, to develop individualized care plans.
• Administer medications, treatments, and personalized interventions as prescribed by physicians.
• Monitor and evaluate patient progress, updating care plans as necessary and communicating changes to the healthcare team.
• Educate patients and their families about their medical conditions, treatments, and necessary self-care techniques.
• Coordinate with physicians, therapists, and other healthcare professionals to ensure a collaborative approach to patient care.
• Provide emotional support and assistance to patients and their families, promoting a comfortable and safe environment within the home setting.
• Document and maintain accurate patient medical records and reports on treatment and progress.
• Adhere to ethical and legal nursing standards and comply with all regulations, policies, and procedures.
• Participate in ongoing training, educational programs, and professional development opportunities to stay updated on best practices and advancements in the field.

Qualifications:

• BSN/Diploma in General Nurse
• Valid nursing license in Pakistan Nursing Council (PNC).
• Proven experience as a Registered Nurse or Licensed Practical Nurse in a home care setting is preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and within a multidisciplinary team.
• Compassionate and empathetic approach toward patient care.
• Proficient in basic computer skills for documentation purposes.
• Understanding of patient confidentiality and privacy laws.

Preferred Skills:

• Experience with various medical equipment and devices commonly used in home care.
• Knowledge of community resources and support services for patients.
• Bilingual or multilingual capabilities may be advantageous.

Working Conditions:

• The role requires frequent travel to patients' homes within the designated area.
• Irregular hours may be necessary to accommodate patient needs, including evenings, weekends, and occasional on-call duties.
• Physical stamina is essential as it involves standing, lifting, and moving patients.

Contract length: 12 months

Salary: Rs45,000.00 - Rs120,000.00 per month
